Social Worker — Hospice, PRN

What you'll do

• Complete comprehensive psychosocial assessments within five days of hospice election
• document needs in the plan of care and clinical records
• assist with advance directives, funeral planning, discharge planning, and family counseling
• provide end-of-life, death, and grief counseling
• attend interdisciplinary team meetings and participate in care planning
• coordinate with the interdisciplinary group
• respect patient privacy and property.
