Remote Care Coordinator – Cardiac

The Remote Care Coordinator – Cardiac role is 100% remote, with the Dallas/Fort Worth area preferred. Pay is $26–$32 per hour. Cardiac Care Alliance works with independent cardiologists to improve access, outcomes, and patient experience through proactive care coordination. The coordinator supports medically complex cardiac patients through structured outreach, follow-up, education, and care-gap tracking. The role works with registered nurses, cardiologists, and clinical partners, escalating concerns while leaving clinical decisions to licensed providers and supervising nurses.

What you'll do

• Conduct structured telephone outreach to cardiac patients
• manage assigned caseloads and prioritize using risk stratification
• complete initial assessments and follow-ups on symptoms, medication adherence, function, and psychosocial needs
• identify social needs and escalate resource requests
• review population health dashboards and address care gaps
• provide patient education and self-management support
• support post-discharge follow-up within 48 hours
• document interventions, care plans, and goals
• communicate with nurses, cardiologists, primary care offices, and clinical partners.

Why this role
