Job overview

What you'll do

• Guide clients in daily living and community-based skills
• support rehabilitation and supported-living goals
• identify changes in behavioral, medical, emotional, cognitive, or physical condition and alert team members
• communicate with clients, families, and the treatment team
• follow behavior plans
• assist with medications as assigned
• maintain a safe living environment
• prepare meals according to nutrition plans
• transport residents to errands, appointments, and outings
• complete required documentation.
