What you'll do

• Perform comprehensive admission assessments
• assess physiological, psychosocial, environmental, and health-history needs
• develop and revise individualized care plans
• coordinate visit frequency and interdisciplinary services
• provide skilled nursing care and patient/caregiver education
• monitor progress and report changes
• participate in case conferences
• supervise the Home Health Aide plan of care every 14 days
• obtain and clarify physician orders
• complete documentation
• coordinate discharge planning
• and refer patients to community resources.
