Hybrid Care Manager

What you'll do

• Identify appropriate patients with providers and practice staff

• monitor health and conduct behavioral health screening
• provide telephone advice and follow-up
• respond to abnormal readings and urgent calls during working hours
• coordinate transitions and referrals
• document progress
• and may conduct home visits with a physician.
