Job overview
The Patient Growth and Community Liaison role is a field-based, onsite position across Lake Zurich and Hoffman Estates, IL, paying $31–$34 per hour for approximately 20–30 hours per week to start. What you'll do
- Build and maintain referral relationships with hospitals, healthcare providers, social services, recovery organizations, criminal justice partners, and families
- educate partners about methadone and buprenorphine treatment
- follow up with referral sources and prospective patients
- connect interested people with intake
- track outreach and outcomes
- and work with intake, operations, and clinical leaders to address barriers to treatment.
