CHILD Waiver Community Living Support

The role provides one-on-one support to children with developmental or intellectual disabilities, helping them build social skills, manage daily routines, and participate safely in community life. It also supports families by implementing person-centered goals, documenting services, and coordinating with case managers and the clinical team.

What you'll do

• Implement goals in each child’s Person-Centered Service Plan
• provide one-on-one supervision during community outings
• support social skill development and daily living activities as needed
• document services and progress in real time
• monitor and report changes in physical or behavioral health
• complete incident reports within required timeframes
• document billable hours according to Kentucky Medicaid and waiver standards
• collaborate with families, case managers, and the clinical team
• maintain a safe environment
• and follow confidentiality and HIPAA requirements.

Why this role

High school diploma or GED; experience with children, youth, or families is preferred but not required. Must have a valid Kentucky driver’s license and vehicle insurance and be able to obtain and maintain First Aid/CPR and CORE training.
