Provide and coordinate care for patients. Collaborate with physicians and multidisciplinary team members to provide physical and psychological support to patients and their families. Perform assessments and reassessments, develop individualized plans of care, administer medications, document care, provide education, maintain equipment, and communicate patient needs.
