Intake Registered Nurse (RN)

What you'll do

• Conduct initial patient assessments and evaluations, completing visits and documentation within 48 hours of assignment
• educate patients and families on home health strategies and procedures
• collaborate with the Intake Coordinator on admissions
• review referral information
• support collection and verification of clinical documentation and physician orders as directed
• communicate with patients, families, and referral sources
• coordinate scheduling and service initiation
• maintain accurate, timely, compliant intake documentation
• identify and communicate clinical concerns or admission barriers.
