Referral Specialist / Community Health Worker

Job overview

What you'll do

• Greet patients, families, and visitors
• screen and schedule referrals
• notify patients of appointments and follow up with off-site providers
• obtain prior authorizations for diagnostic services, procedures, and specialty medications
• work with clinicians to address patient needs
• assist with medication assistance programs
• maintain medical information in systems
• answer questions and support front desk and medical records as needed.
