What you'll do
- Participate in referral matching
- prepare apartments for intake
- develop treatment plans and coordinate assessments
- obtain required legal documentation and safety plans
- monitor treatment plans and client progress
- coordinate with placement agencies, schools, and community resources
- provide training and support for independent living
- complete apartment inspections and documentation
- participate in crisis prevention and management, including a weekly rotating on-call schedule
- enter required records in the electronic health record.
