What you'll do

• Perform comprehensive and ongoing assessments
• complete Start of Care, recertification, reassessment, and OASIS visits
• develop and update care plans
• provide skilled nursing under physician orders
• monitor patient status and report changes
• coordinate with the care team
• supervise LPNs and CNAs as applicable
• maintain timely, accurate clinical documentation.
