What you'll do

• Coordinate and oversee the RAI/MDS process and timely submission of assessments
• ensure compliance with professional standards, physician orders, facility policies, and regulations
• develop individualized resident care plans
• collaborate with nursing, therapy, dietary, social services, physicians, residents, and families
• facilitate care plan conferences and applicable Quality Measures meetings
• oversee assessment validation reports.
