What you'll do
- Coordinate scheduling and delivery of skilled nursing and therapy services
- process physician orders, referrals, and authorizations
- conduct patient assessments and assist with individualized care plans
- communicate with clinicians, physicians, patients, and families
- monitor progress and update care plans
- ensure documentation meets Medicare and Medicaid requirements
- participate in quality improvement
- support field nursing staff
- resolve authorization and documentation issues with billing and administrative teams
- contribute to case conferences.
