Director of Care Management

What you'll do

• Provide strategic leadership and operational oversight for Integrated Care Management programs across hospital and ambulatory settings
• lead teams including RN Care Managers, RN Care Coordinators, and Licensed Social Workers
• build partnerships with clinical leaders, community organizations, and cross-functional stakeholders
• drive process improvements to reduce readmissions, improve patient flow, optimize length of stay, and improve care coordination outcomes
• ensure compliance with federal and state requirements
• coach, mentor, and develop team members
• analyze clinical, operational, and financial performance metrics.
