The Client Care Coordinator — Front Range role is onsite in Broomfield, CO. Pay ranges from $22.40 to $25.76 per hour. The position supports clients receiving treatment at Foundry Front Range and reports to the Clinical Director. The coordinator provides ongoing client support and develops continuing care plans to help clients transition safely after treatment. The role connects clients and families with clinical recommendations, appointments, transportation, and continuing care resources while coordinating with internal and external teams.

What you'll do

• Assess client needs and barriers to care
• determine post-treatment programs considering clinical, financial, and family factors
• develop continuing care plans with clients and families
• coordinate discharge logistics, appointments, and transportation
• manage schedules
• collaborate with Admissions, Medical, Wellness, Family, and Clinical teams
• document accurately
• support client safety and confidentiality
• coordinate with continuing care facilities and verify benefits.

Why this role

Bachelor’s degree preferred; high school diploma or equivalent required. Two or more years of clinical-setting people and program management experience; CPR certification must be obtained within the first 30 days. Valid, non-restricted Colorado driver’s license required.
