Advanced Practice Provider (APP) - Cardiothoracic Surgery

Provide clinical care to patients transitioning from the Cardiac Surgery ICU to the step-down setting. Evaluate and develop plan of care for new patient consults. Perform detailed assessments, monitor/evaluate cardiac status, manage patient through the pre-post-op period, initiate/refine medical management. Participate in multi-disciplinary rounds, care coordination, discharge planning and educati
