What you'll do

• Provide skilled nursing care under individualized hospice plans
• assess physical, emotional, and comfort needs
• manage pain and symptoms
• administer medications and treatments and evaluate responses
• communicate changes to the clinical director, physicians, and interdisciplinary team
• educate and support patients, families, and caregivers
• coordinate services
• participate in care planning and team meetings
• document care using Kinnser/WellSky
• maintain regulatory and safety compliance
• travel to patient visits throughout the South Bay.
