What you'll do

• Accept and treat patients within the designated geographic area
• evaluate home-based physical therapy needs and develop individualized treatment plans
• establish goals in consultation with physicians, patients, and families
• plan and implement therapy programs to restore function and promote independence
• instruct caregivers in therapy techniques
• document each visit
• communicate with patients, families, and interdisciplinary team members
• comply with agency policies and professional standards
• travel to patients’ homes within the assigned territory.
