The position initially begins as PRN and is expected to ramp to full-time as client volume grows. Collage Home & Community Rehabilitation Partners supports Texas licensed home health services for medically complex patients. The Registered Nurse provides clinical oversight of nursing and aide services, develops and monitors care plans, and supports regulatory compliance and quality improvement. The role also helps establish scalable clinical workflows as the program grows and new clients are onboarded.

What you'll do

• Supervise nursing and aide services under the Texas LHHS license
• provide clinical supervision of RNs, LVNs, and CNAs
• ensure CNA services are assigned, delegated, and supervised appropriately
• conduct or oversee patient assessments
• develop, review, and update physician-ordered plans of care
• monitor patient progress and respond to changes
• maintain clinical documentation
• participate in audits, surveys, and quality improvement
• support client onboarding and scalable clinical workflows.
