What you'll do

• Identify, coordinate, and assure delivery of supports and services
• determine eligibility under state guidelines
• provide information and support to families
• help clients obtain services and benefits, transportation, and community involvement
• monitor appointments, progress, and service delivery
• maintain case notes, treatment plans, files, and reports
• provide supportive and problem-solving counseling
• and respond to crisis situations.
