The Hospice Registered Nurse Case Manager role is onsite in New Brunswick, NJ. Compensation is $90,000–$95,000 per year. Swan Hospice provides care to patients in skilled nursing facilities, assisted living facilities, and private homes. The nurse manages a primary hospice caseload and coordinates skilled nursing care for patients, families, and caregivers. The role assesses and reassesses nursing needs, develops and updates plans of care with the Hospice Interdisciplinary Team, and teaches, supervises, and counsels those involved in patient care.

What you'll do

• Oversee a primary hospice caseload
• conduct initial and comprehensive nursing assessments and identify patient needs
• collaborate with the Hospice Interdisciplinary Team on plans of care
• implement, evaluate, and revise care plans
• teach, supervise, and counsel patients, families, caregivers, and staff
• coordinate with the interdisciplinary team
• supervise hospice aides and homemakers.
