What you'll do

• Develop, implement, and evaluate care management policies and objectives
• support care coordination and patient care plans
• conduct clinical assessments and develop integrated plans
• support patient education materials
• monitor and train assigned staff
• assist with post-hospital transitions
• participate in team rounds, data tracking, complaint resolution, and committees.
