What you'll do

• Provide nutrition assessments for admissions, quarterly and annual reviews, tube feedings, and significant changes.
• Create and update care plans for interdisciplinary teams.
• Conduct nutrition-focused physical exams to identify malnutrition.
• Participate in care conferences and clinical meetings.
• Perform monthly kitchen inspections and meal service audits.
