Hospice Registered Nurse Case Manager

What you'll do

• Assess patients and families
• provide nursing care and develop, initiate, and revise care plans
• administer prescribed medications and treatments
• counsel and educate patients and caregivers
• document care and communicate changes to physicians
• coordinate with the hospice interdisciplinary group
• supervise ancillary personnel as required
• participate in on-call duties and be available every other weekend.
