What you'll do

• Receive and triage patient and family calls according to clinical urgency and needs
• provide customer service and clinical guidance
• assess needs and coordinate services under the established plan of care
• communicate changes to clinical staff
• coordinate changes in level of care with schedulers and other departments
• document assessments, interventions, observations, and communications
• maintain call logs and share pertinent information with the interdisciplinary team.
