What you'll do
- Assess clinical stability and home readiness for HHD and PD
- provide in-home training and ongoing clinical support
- teach equipment use, infection prevention, troubleshooting, and emergency procedures
- validate patient and caregiver competency
- collaborate on individualized plans of care
- monitor outcomes, adherence, and clinical indicators
- identify risks and intervene
- comply with CMS ESRD Conditions for Coverage, CDC, OSHA, and organizational policies
- maintain clinical documentation
- participate in QAPI, survey readiness, audits, and corrective actions
- support care coordination and dialysis continuity during emergencies.
