Street Health Outreach Medical Assistant

The Street Health Outreach Medical Assistant role is hybrid, combining field-based outreach and clinic-based work across San Diego County; pay is $24.00–$27.61 per hour. The Street Health Program supports people experiencing homelessness through outreach, engagement, and integrated healthcare. The role exists to build trusted relationships and connect people to healthcare and support services while assisting with clinical care and care coordination. It contributes to patient engagement, continuity of care, and health outcomes through field outreach, clinic support, and collaboration with interdisciplinary teams.

What you'll do

• Conduct community outreach
• maintain an assigned caseload including Enhanced Care Management members
• take vital signs and document patient information
• assist licensed clinicians with patient care
• coordinate care across healthcare, housing, behavioral health, and social services
• complete timely electronic health record documentation.
