What you'll do

• Support Lead Care Managers with workflows, questions, productivity, and documentation
• assist with onboarding, shadowing, and coaching
• provide direct member support and coverage as assigned

• assist with transitions of care and follow-up
• participate in community outreach
• maintain compliant documentation and support quality improvement.
