What you'll do

• Assess service needs and make referrals
• obtain and renew physician orders
• develop, interpret, and update client plans of care
• visit clients’ homes to assess needs and environments
• review client condition and personal care worker records at least every 60 days
• instruct personal care workers and arrange demonstrations of services
• travel to client homes and report care status and recommendations to a supervisor.
