What you'll do

• Provide caregiver and family support, education, resource navigation, and care-planning consultation
• develop and facilitate support groups
• build relationships with healthcare providers and community organizations
• conduct outreach and community presentations
• connect participants with services and care options
• facilitate evidence-based programs and maintain required certifications
• document contacts, referrals, services, and outcomes
• track participation and satisfaction
• represent Active Generations at meetings and events
• travel throughout the assigned service area.
