Hospice Chaplain | Bereavement Coordinator

What you'll do

• Complete spiritual assessments within five days of admission
• collaborate with the interdisciplinary team on care plans
• provide spiritual support aligned with patient and family beliefs
• liaise with community clergy when requested
• offer prayer, worship, and rituals
• document assessments and visits
• participate in IDG and QAPI meetings
• support staff and bereavement services
• oversee bereavement risk assessments and follow-up plans
• coordinate support for up to 13 months after a patient’s death
• lead funeral or memorial services upon request.
