What you'll do

• Provide comprehensive assessments, telephonic check-ins, and in-person visits to high-need patients.

• Maintain knowledge of community resources and make referrals.
• Navigate patient insurance benefits.
• Provide psychoeducation on life stage and disease-specific issues.
• Meet with care teams to collaborate on patient issues.
• Complete documentation on time.
• Partner with Community Health Workers.
• Respond to crises.

What we're looking for

Education

Master of Social Work

Experience

Master of Social Work degree. Minimally licensed in the state. Proficient computer skills. Fluency in Spanish, Polish, Russian, or other languages where necessary.
