Care Coordinator

Job overview

The Care Coordinator role is onsite in Athens, TN; compensation is not specified. Omni Family of Services provides foster care, adoption, outpatient behavioral health, and community-based family support, with a focus on trauma-responsive care for children, adults, and families. The coordinator works with clients and their families to address service and advocacy needs through scheduled home or residence visits, phone triage, and, when specified, meetings with extended family and community supports. What you'll do

• Meet regularly with clients and their families in the home or residence
• conduct phone-call triage assessments during off months and weeks of face-to-face care
• and, as specified to meet service and advocacy needs, meet with extended family and community supports at locations such as schools, courts, or job sites.
