What you'll do
- Assist with developing and revising individualized care plans
- respond to staff and physician calls about orders or care concerns
- ensure accurate entry of orders, medication updates, and patient information
- lead weekly case conferences
- maintain referral relationships
- enter referrals in Home Care Home Base
- assist with scheduling and occasionally provide direct patient care
- participate in hiring, evaluations, and disciplinary actions
- practice infection control and report suspected abuse.
