What you'll do

• Guide treatment plans with the interdisciplinary team
• coordinate tentative discharge and contingency plans
• facilitate weekly team conferences and treatment plan changes
• complete case management documentation
• meet with patients and families within 24 hours of admission and complete assessments within 48 hours
• arrange resources and referrals
• support utilization review and participate in the case management on-call schedule as needed.
