Job overview

What you'll do

• Assess patients and families and determine home care needs
• reevaluate nursing needs
• initiate and revise plans of care with physicians
• provide preventive and rehabilitative nursing procedures and prescribed medications and treatments
• counsel and instruct patients and families
• coordinate discharge planning
• document care
• communicate patient changes and needs to physicians and other care providers
• participate in on-call duties
• arrange equipment and supplies
• supervise home health aides and LPN/LVN care.
