What you'll do

• Provide hands-on patient care at assigned homes or facilities
• follow individualized care plans
• assist with activities of daily living, transfers, ambulation, and other care needs
• support skin integrity through repositioning and related care
• report changes or observations to the nurse supervisor
• perform delegated tasks such as vital signs and weights
• follow safety and infection-control procedures
• participate in the on-call caregiver rotation.
