Community Health Worker — Home and Facility Visits

What you'll do

• Coordinate care with team members and follow care plans
• facilitate visits with providers, therapists, and other services
• deliver medications and reconcile medication lists
• coordinate telehealth visits
• train patients on remote patient monitoring equipment
• deliver and set up durable medical equipment and instruct patients and caregivers
• perform routine venipuncture and collect and handle specimens as needed.
