What you'll do

• Provide skilled nursing visits under established care plans
• conduct physical, psychosocial, and environmental assessments
• monitor health status
• collaborate with physicians, therapists, and interdisciplinary team members
• report changes and adjust care plans as needed
• communicate with patients, families, and caregivers
• educate on disease management, medications, treatments, safety, and infection prevention
• document in the EMR
• follow agency policies, HIPAA, and regulatory requirements
• participate in training, case conferences, and quality improvement.
