What you'll do

• Assess patients and families to determine home-care needs
• reevaluate nursing needs and revise plans of care
• use assessment data to determine nursing diagnoses
• develop goals and therapeutic, preventive, and rehabilitative actions with patient and family involvement
• administer prescribed medications and treatments
• counsel and educate patients and families
• identify and implement discharge-planning needs
• coordinate assigned caseloads when acting as case manager
• prepare clinical notes and communicate with physicians and community health providers
• participate in on-call duties
• arrange necessary equipment and services
• instruct, supervise, and evaluate home health aides every two weeks.

Why this role
