What you'll do
- Manage admissions from referral through placement
- coordinate stakeholder communication and admissions documentation
- monitor timelines and performance metrics
- facilitate meetings and transitions
- prepare reports and track key metrics
- conduct post-admission follow-ups
- guide families on DDA eligibility and applications
- develop and maintain a person-centered planning quality improvement system
- coordinate assessments and planning documentation.
