Patient Advocate / Social Worker

The Patient Advocate / Social Worker role is onsite in Midland, TX, at a new clinic scheduled to open January 4, 2027. Compensation is $65,000–$75,000 per year. The role supports Proactive MD’s workplace health center and reports to the Director of Patient Care Navigation. The position exists to connect health-center services with community care and help employees and dependents navigate treatment. It coordinates referrals and records, supports patient engagement and education, and works with providers and clinical operations to promote continuity of care.

What you'll do

• Coordinate care with external specialists, imaging centers, labs, equipment providers, counselors, and community resources
• ensure records are exchanged for specialist visits
• receive prior authorizations and send notifications as appropriate
• help patients navigate healthcare
• coordinate emergency-room and hospital transitions
• engage patients and coordinate follow-up
• support outreach and education
• provide information about community services and insurance benefits
• collaborate with clinical operations and providers.

Why this role
