Job overview

The Health Partner role is onsite five days a week in Providence, RI, with compensation of $50,000–$60,000 per year. Daymark Health provides personalized oncology care navigation, symptom support, behavioral health care, and social resources. The Health Partner coordinates social and logistical aspects of patient care, connecting patients with community resources and helping arrange follow-up and referrals. The role supports integrated care by keeping patients, caregivers, providers, and external services coordinated.

What you'll do

• Conduct structured social needs assessments and connect patients with community resources
• support care plans and case conferences
• prepare patients for virtual visits and provide basic technology troubleshooting
• schedule follow-up visits and facilitate referrals
• prepare charts for providers
• track symptom surveys, follow up, and escalate clinical concerns
• document interventions and coordination in the electronic health record.

Why this role
