What you'll do

• support care planning, patient assessments, and coordination of clinical services
• ensure adherence to agency policies, state regulations, and professional standards
• assist with staff training and competency evaluations
• monitor patient outcomes and communicate changes
• act as Clinical Manager when delegated or during an absence
• maintain compliance with home health regulations.
