What you'll do

• Assist with planning meaningful daily activities
• support clients with social and other concerns
• maintain client records and submit incident reports
• implement individualized plan goals
• accompany or transport consumers to programs and activities
• monitor progress and safety
• support daily living, social, vocational, and community skills
• participate in meetings and required training.
