The Care Manager role is onsite in Reno, NV. Pay is $58,240–$66,560 per year at 40 hours per week. The role is the first Care Manager position at HomeWell Care Services’ newly licensed Reno operation, helping establish its assessments, care plans, family relationships, and operating standards. The Care Manager supports clients and caregivers through home assessments, service planning, supervisory visits, and family communication. The role contributes to quality and compliant home care while helping families respond to changing needs; scheduling remains a separate role, with backup support provided when needed.

What you'll do

• Meet prospective clients and families in their homes and assess needs
• develop and revise service plans
• conduct supervisory visits and observe caregivers in the home
• coach caregivers where helpful
• respond to families when circumstances change
• maintain documentation for state surveys and support quality and compliance
• serve as Administrator Designee
• back up the scheduler when needed
• participate in a shared on-call rotation.
