The Visiting After-Hours Registered Nurse (RN) role is primarily work-from-home, with visits to participants’ homes or communities when medically necessary, in the Orlando, FL service area. The nurse responds to after-hours calls, assesses and triages participant needs, and determines appropriate interventions, including education, treatment, or a home visit. The role helps maintain continuity of care for participants through clinical judgment, coordination with providers and care teams, and timely documentation.

What you'll do

• Respond to after-hours calls from participants, families, or staff

• support care transitions and proactive visits
• document care in the electronic medical record
• hand off high-risk participant information to peers and center teams.
