What you'll do

• Oversee the implementation and assessment of patient care plans.
• Collaborate with spiritual care coordinators, social workers, hospice aides, and medical directors.
• Communicate patient conditions and adjust care plans as needs evolve.
• Provide one-on-one patient care.
