What you'll do

• Assess patient needs

• coordinate appropriate services with patients, families, healthcare providers, and care team members
• support continuity of care and effective resource use
• perform acute-care case management, discharge planning, and utilization review. Requires an active Virginia or compact RN license, BLS, EPIC charting proficiency, and recent acute-care hospital case management experience
• weekend coverage is every third weekend, with day shifts of five 8-hour days, 08:00–16:30.
